Cervical spondylotic myelopathy (CSM) is an age-related spondylotic change that may result due to compression and ischemic dysfunction of the spinal cord. It manifests symptoms like neck stiffness, one-sided or both sided pain in the neck, arm, and shoulder, stiffness while walking, etc. The allopathic management for CSM is mainly a symptomatic treatment and does not ensure complete cure. Among various herbal remedies, a case of CSM was treated with Panchakarma procedures and ayurvedic oral medicines. The patient was suffering from neck stiffness (greevastambha) and treated with sudation with medicated cooked bolus of rice for one month and enema with medicated milk for 16 days along with oral ayurvedic drugs like Brihatavata chintamani rasa, Ekangaveer ras, Ardhangavatari rasa, Amrita satva(dry extract of Tinospora cordifolia Willd), Muktasukti pisti, Ashwagandha churna(powder of Withania somnifera Dunal), Dashmool kvatha ghana (solid extract of Dashmoolkvatha), Trayodashanga guggulu, and twice a day with honey and Eranda paka twice a day with milk. Later the patient showed positive results and improvement in the condition of CSM.
